FACILITY INFORMATION: Please complete the form to request a building or facility reservation. Once your request has processed, you will receive an e-mail notification confirming your reservation including any details. Administration reserves the right to deny and/or change any reservation request at any time. Rental and custodial fees may apply.
VEHICLE INFORMATION: For vehicle reservations you will need to select VEHICLE FLEET in the LOCATION and select a vehicle that you'd like to use. ATTENDANCE is a required field and means the number of people traveling, including the driver. Please enter your DESTINATION in the SET UP NOTES. Once your request has been processed, you will receive an e-mail confirming your reservation and letting you know which vehicle has been assigned to you.
* Return keys and vehicle to MHS immediately upon return.
* Return completed mileage log sheet to Meagan Schoenfelder at the MHS office.
* Return gas receipt to your building supervisor/office.
* If you need keys before 7:30 a.m. please remember to pick them up the DAY BEFORE - it is your own responsibility to get keys on time.
* All vehicles must have a full tank of gas upon returning. Gas Code: 57301
* MTI Employees should use a credit card from your business office for fuel.
Keys can be picked up from the High School office during regular business hours (7:30 a.m. – 4:00 p.m.). Complete the vehicle log each time you use it, noting any issues that need attention. Please note that all gas expenses will be charged to your department or student/staff travel expense. Gas receipts need to be returned to your building supervisor. Please return vehicle keys and log sheet immediately following your return. All vehicles are located in the east parking lot of MHS. There is a mail slot key drop box located on the garage door that faces the east parking lot for after-hours convenience, if needed.
**REMEMBER WHEN USING THE MINI-BUSES: The emergency back door latch MUST be open/unlocked while driving and locked when parked. A loud buzzer will sound if you do not have this done correctly and prevent you from driving.**
PLEASE KEEP IN MIND THAT VEHICLES ARE NOT ASSIGNED ON A FIRST COME FIRST SERVE BASIS BUT RATHER ASSIGNED BY NEED BASED ON DISTANCE, NUMBER OF PEOPLE TRAVELING, AND MANY OTHER FACTORS. YOU MAY BE ASSIGNED A VEHICLE BUT LATER THAT MAY CHANGE DUE TO AVAILABILITY. WE DO OUR BEST TO ACCOMMODATE THE BEST WE CAN WITH THE VEHICLES WE HAVE. Thank you for your understanding.
